Please complete this form if you would like to request a change in your child's learning option. The learning option may not be available immediately. The change must be approved by an RRMS administrator and will be confirmed by email.

AS THE TRANSFER DEADLINE HAS EXPIRED, YOU MAY COMPLETE THE FORM AND PROVIDE AN INTENT TO RETURN WITH A SPECIFIC DATE (MONDAY ONLY). THE RRMS ADMINISTRATION WILL CONTACT YOU TO FOLLOW UP.
